In order to make full use of this service, you will have to provide some personal information relating to you. Before you submit any information, you should be aware of how it will be used. By providing your information, you agree to the collection, use and disclosure of your personal information as per the provisions below. 1. Data Privacy
1.1 GlaxoSmithKline Services Unlimited (“GSK”) is the Data Controller. The information will be collected and stored by GSK’s appointed agency (the Data Processor). GSK will only have access to some of the information in aggregate and non-identifiable format.
1.2 Your information will be used in to order provide you with access by email and through notifications delivered by the applications and website. GSK may also use aggregated and non-identifiable data collected from the users of this Application for research purposes.
1.3 GSK and the Data Processor will use commercially reasonable means to ensure that your personally identifiable information is protected appropriately according to the sensitivity of the information. You agree that your data may be transferred outside of the European Union and thereby subject to different legal requirement than the European privacy laws.
1.4 GSK will not sell or rent personally identifiable information to third parties. Sometimes selected third parties are used to provide GSK with support services and such parties may, from time to time, have access to your information to enable them to provide those services to GSK.
1.5 If you would like to be removed from MyAsthma, please use the delete account function which is found in Settings.
1.6 If you choose to record your full name or other information that directly identifies you, we recommend that you password protect your mobile device or computer upon startup to protect your personal information.
2. Reporting of Side Effects
2.1 If you have a side effect to report for any GlaxoSmithKline product, please contact us on 0800 221 441.
2.2 If you have a side effect to report for a product manufactured by another manufacturer, you should contact the manufacturer of that product – relevant contact details should be found on the Summary of Product Characteristics.
2.3 Information about adverse reactions to medicines and reporting forms can be found at www.yellowcard.mhra.gov.uk.
